The objective of this RFP is to partner with a skilled nursing facility (SNF) provider to create a 10-16 bed high acuity care unit at 1-2 Skilled nursing facilities in Portland, Oregon, to improve early access to and coordination of post-acute rehabilitative care for OHSU patients with the goals of opening OHSU acute care occupancy for Oregonians in need for acute hospital admissions.
