The Provider shail implement programs and activities within communities that address the root causes of COVID-19 or address the social determinants of health that are unique to the community by advancing health equity, by creating the resource(s) needed to address these broader needs and by providing services in culturally relevant, linguistically appropriate, and timely ways.
Applicants will identify a potential issue within a clinical diagnostic or group of clinical diagnostics that perpetuates health inequity and disparities, clearly define the disparity issue, and develop a solution to address the concern.

Additional funding is relevant to current CDC REACH grant recipients that work to address health disparities at the community level. 2.0 PROJECT OBJECTIVES Specific objectives are to increase awareness of: • Flu prevention and symptoms • Flu vaccine safety and efficacy • Similarities and differences with COVID19
The goal of the CSHS Affiliated Programs is to build on existing infrastructure to provide all school-aged children access to a quality school health program that is managed by a qualified school nurse manager. This program will provide access to clinical consultation and professional development through both regional and statewide programs in order to improve clinical school nursing practice and address health disparities and racial inequities.
