This Request for Proposals (RFP) is published to solicit proposals for the provision of services to increase access to integrated healthcare for the Maternal and Child Health (MCH) target population of Women, Infants, Children and Adolescents from birth to 21 years of age, and to address the Maternal, Children and Youth health priorities as identified in the State’s Maternal and Child Health five (5) year Statewide Needs Assessment completed in 2020
Individualized care shall take into consideration the realities of a youth’s life, understanding her/his life in context as an effort to address the etiology of the youth’s symptoms and behaviors rather than seek to simply “contain” them. Individualized care shall address the unique experiences of the youth.
The goal of treatment is to engage youth to address clearly identified needs, stabilize symptomology, and prepare the youth for a less restrictive environment. B. Informal Counseling • Support individualized goal setting with children/youth and their families to support health goals and address identified needs. • Describe to children/youth and families the purpose of health care services/recommendations and provide tailored education regarding effective use of the local healthcare and social services systems. • Provide culturally appropriate health education and wellness coaching on topics related to prevention
The model staffing structure includes supports to address integrated health components of care necessary for the identified population, such as medical, psychiatric, behavioral, communication, social and emotional, family dynamics, and respite.
