The Department of Health, Division of Senior and Disabilities Services, is entering into Agreements under 7 AAC 81 with qualified Assisted Living Home Providers for the State of Alaska’s General Relief Assisted Living Home Provider Agreement program to provide eligible adult Alaska residents assisted living home services.

Eligible consumers receive Long Term Supports and Services. These Long Term Supports and Services are provided in accordance with each consumer’s Plan of Care. They may include essential services such as service coordination, meals, medication administration, assistance with personal care, and chore or homemaking assistance.
