Behavioral Health Care Services for Pleasanton Residents Description The City of Pleasanton is seeking a provider(s) to address local service gaps related Behavioral Health. Grant goals: promote collaborative, integrated primary and behavioral health care, defined as a whole person approach to health and wellness that makes receiving care seamless for the patient and family. Services include brief mental health interventions in primary care offices, consults, intensive care coordination, and health promotion activities such as wellness, child yoga and health coaching.

The purpose of this grant will be to: (1) promote full integration and collaboration in clinical practice between primary and behavioral healthcare in 2-4 counties in Vermont and support the improvement of integrated care models for primary care and behavioral health care to improve the overall wellness and physical health status of children with a serious emotional disturbance (SED).
