Outreach program will identify, assess, and refer homeless individuals in the City to appropriate housing programs and services. The team will address a person’s mental health, substance use disorder, and/or medical needs by building trust between the individual and the service teams and identifying opportunities through which other services can be provided.

Such proposals must be filed with the Ontario County Purchasing Director, 20 Ontario Street, Canandaigua, New York 14424, on or before 4:00 PM prevailing time April 29, 2024. All Delaware local education agencies (LEAs) are eligible to apply. Only Delaware LEAs are eligible to apply.
