While there are other vendors capable of performing Customer Service Survey card collection, until the remainder of the current survey cards are collected and distributed to MDEO s chosen recipients, another vendor cannot perform this work.

The past performance evaluation may be based on the contracting officer s knowledge of and previous experience with the supply or service being acquired; Customer surveys, and past performance questionnaire replies; Contractor Performance Assessment Reporting System (CPARS) at http://www.cpars.gov/; or any other reasonable basis.

Janitorial services requested for Helena, MT Air Traffic Control Tower (ATCT).Please see attached: solicitation, statement of work (SOW), 3 frequency of service charts (FOS), wage determination, contractor staffing access questionnaire, customer satisfaction survey, and procurement schedule.

REPORTING REQUIREMENTS: The COR shall distribute surveys to beneficiaries, using feedback as a measure of customer satisfaction. The Contractor shall monitor transportation log information and customer surveys for timeliness and level of satisfaction with customer service. The Contractor shall respond to each report of unsatisfactory service within five (5) working days of receipt of the written report.

Vendor will implement a customer satisfaction survey within the first three (3) months of contract start date which is electronic in format, accessible via computer and smartphone. Satisfaction surveys shall be able to be completed in less than five (5) minutes. BCVSC will have access to download all results. Questions included in the survey will be negotiated with the successful Vendor.
