REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 19, Rocky Mountain Acquisition Center (RMAC) is seeking information and sources that can provide Oklahoma City VA Medical Center with a cytology prepping cassette printer that is LIS compatible with VISTA and CERNER.
Department of Veterans Affairs requires the contractor to provide the San Francisco VA Health Care System (SFVAHCS) with Cytology services. The proposed contract action is to perform these services for Pathologists to provide diagnosis for VA Beneficiaries. This acquisition is conducted under the authority of FAR 13.106-1. Only One Responsible Source and No Other Support or Services Will Satisfy Agency Requirements.
LAB REPORTS: All lab reports shall contain at a minimum the following information: Patient s full name Physician name Type of specimen Patient s Social Security Number Patient Date of Birth Requesting Facility REPORTING OF LAB REPORTS RESULTS: All lab reports shall be reported via fax, and a hard copy of all lab results shall be sent via mail.
The Contractor shall propose the following cytology tests type. Description of Tests (Stains/ Specimens) Estimated Qty.
The purpose of this notice is to identify potential sources to provide cytology/pathology laboratory testing and services to the Laboratory Department at Pinon Health Center facility.
