This procurement is being conducted in accordance with FAR 13.501 in accordance with 41 U.S.C. 1901 only one responsible source and no other supplies or services will satisfy the agency requirements. The contract will be firm-fixed-price, and the NAICS Code is 621511. Department of Veterans Affairs requires the contractor to provide the San Francisco VA Health Care System (SFVAHCS) with Cytology services.
The Contractor shall propose the following cytology tests type. Description of Tests (Stains/ Specimens) Estimated Qty.
Contractor shall perform and provide results for, but not limited to, the following procedures/tests: CPT Code Service Description Est. Annual Quantity 034521 Cytology Pap Gyn 2160 034004 HPV High Risk 1150 Contractor shall provide laboratory services to the Pathology & Laboratory Medicine Service of the VA North Texas Health Care System, Department of Veterans Affairs Medical Center, located at 4500 S. Lancaster Road, Dallas, Texas.
In addition to on-site services, the contractor will provide off-site professional consultation as needed. Off-site professional services will include pathologists with subspecialty fellowship training in Dermatopathology, Cytopathology and Hematopathology. Off-site services will include all cytology services (excluding GYN and urine cytology), both pathologist examination and diagnosis, and technical staff.
This Sources Sought Notice is for the Navajo Area Indian Health Service, Pinon Health Center issued in accordance with FAR 5.101. The purpose of this notice is to identify potential sources to provide cytology/pathology laboratory testing and services to the Laboratory Department at Pinon Health Center facility.
