It is the intent of the Lake County Health Department and Community Health Center (hereinafter “the Health Department”) to award Agreements for Dental Laboratory Services for the Health Department for Lake County. The Health Department reserves the right to award Agreements to multiple Contractors as a result of this RFP and to add or delete additional dental clinic site locations.    

The purpose of this RFP is to enter into Agreements with a Contractor to provide dental laboratory services for the patients of the Health Department.

The Navajo Area Indian Health Services, Kayenta Health Center Dental Department is requesting for a contractor to provide Dental Laboratory Service - Prosthodontics with opportunity to exercise Base Year Plus (2) Option Years.
