Services shall be performed in accordance with the following scope of work and terms and conditions contained herein.The regional treatment center (RTC) (Desert Visions Youth Wellness Center is a 24 bed residential treatment center and Nevada Skies Youth Wellness Center is a 16 bed residential treatment center), is accredited by the Joint Commission.
It is the intent of the Phoenix Area Indian Health Service to acquire onsite dietary and nutritional services under a non-personal service contract for the Phoenix Area Indian Health Services (IHS) Desert Visions Youth Wellness Center located in Sacaton, AZ; and virtual, and onsite dietary and nutritional services for the Nevada Skies Youth Wellness Center located in Wadsworth, NV.
Additionally, the source shall provide applicable rates associated with the services and work located in: Desert Visions Youth Wellness Center 198 Skill Center Rd, Sacaton, AZ 85147. Sources should note that if no service suppliers can be found that Government may as a follow-on issue a Sole Source in accordance with Federal Acquisition Regulation Part 13.105 Synopsis and Positing Requirements for Commercial Services.
The proposed contract action is to provide non-personal professional REGISTERED NURSING SERVICES FOR THE DESERT VISIONS YOUTH WELLNESS CENTER LOCATED IN SACATON, ARIZONA AND NEVADA SKIES YOUTH WELLNESS CENTER LOCATED IN WADSWORTH, NEVADA in accordance with the Scope of Work (SOW). Proposals are being requested and a written solicitation SF1449 is hereby issued.
