The purpose of the contract is for ONC is to obtain information from health care providers and patients, through qualitative research and qualitative analysis to understand:(1) To what extent do health care providers document and use SDOH information to treat a patient or offer referral services(2) How do health care providers leverage open text fields to document SDOH data, specifically clinical notes and assessments section of an EHR to document
BI – business intelligence, VA Arches/MDClone, VA Common Operating Platform, Rockies, amongst others), VA FHIR-based APIs, and VA data sets (i.e. Patient-Generated Health Data, Corporate Data Warehouse, VistA data, Cerner, Community Care Data, Financial Services Center resource and claims date, Social Determinants of Health Data amongst others).
These technologies may include efforts to improve and standardize data collection and the integration of social determinants of health (SDOH) and other data across disparate data sources, including clinical patient data, electronic medical records, public health data, census data, housing data, employment data, and crime statistics.
This includes design, development, and deployment of a novel and/or integrated database and algorithm platform, within or in collaboration with Praedico, with optimized user experience/dashboard(s) to: Visualize the integrative data scores/reports, insights and projections incorporating a) Veteran health data from the VHA database, b) comprehensive climate data elements, and 3) comprehensive social determinants of health data elements, and associated
Regionalization was supported by the 2019 Community Health Needs Assessment which is conducted every 3 years. This assessment provides an opportunity for residents to inform data collection efforts about health issues, social determinants of health, community strengths and opportunities impacting their communities which is then used to generate Health Improvement Plans.
