IRIS is Wisconsin’s self-directed long-term care program for frail elders and adults with physical or developmental disabilities to get the services they need to remain in their homes whenever possible and remain independent.
Solicitation Description: The Health and Human Services Commission (HHSC) on behalf of The Department of Family and Protective Services (DFPS or Department) Adult Protective Services (APS) is issuing this Open Enrollment and seeks to enter into contracts with qualified Applicants to provide Personal Assistance Care (PAC) services to DFPS clients.

The services being funded are: Assistive Technology, Caregiver Services, Case Management, Congregate Meals, Culturally Appropriate Information and Referral, Elderly Legal Assistance Program (ELAP), Home Delivered Meals, Homemaker, Home Modifications and Repairs, Kinship Care, Material Aid, Personal Care, Senior Recreation, Consumer Directed Care (Support Options), and Transportation.
