Contract HE125422D0010 was awarded as a sole source firm-fixed-price (FFP) contract pursuant to FAR 13.106-1(b)(1) to Credentia Nurse Aide LLC. The contract was awarded with a one-year base period and four 1-year option periods.Scope of Work: The Contractor shall facilitate testing procedures and provide exam results to NCDPI for addition to the North Carolina CNA Registry. Currently DoDEA has five high schools (Lejeune, Humphreys, Guam, Kaiserslautern, and Ramstein) that offer the Therapeutic Services - Patient Care pathway. DoDEA anticipates this number of schools to grow in the coming years. See Special Notice HE125422Q0020 published on March 17, 2022 for additional details.
The vendor will provide transport as requested by Island health, on an as needed bases, for Island health staff and patients and will also be used to provide the transport of small items between sites; All Island health requested trips will require an Island health authorized taxi voucher.
The vendor will occasionally be used to provide the transport of small items between sites and for transportation of Island Health staff with authorized taxi voucher. The taxi stand service will be provided for the Comox Valley Hospital. The taxi service for the site would be expected to pick up within a 60 km range of the hospital and drop off at any destination.
