The County, through DHHS’s Aging and Disabilities Services (A&D), wishes to provide Home Health Aid Services. The target population for services provided under this Request for Proposals (RFP) are eligible frail seniors, and adults with physical and mental disabilities.
Solicitation Description: The Health and Human Services Commission (HHSC) on behalf of The Department of Family and Protective Services (DFPS or Department) Adult Protective Services (APS) is issuing this Open Enrollment and seeks to enter into contracts with qualified Applicants to provide Personal Assistance Care (PAC) services to DFPS clients.
In support of the goal to promote healthy parent-child relationships, HFO home visitors (HVs) are trained to use an infant mental health approach which supports the formation of an alliance between the parents and the home visitor and provides tools to promote successful parenting.
To this end, the WSIB is looking at evidence based, program models, (that may or may not involve partnerships), to deliver Home Care Services within the changing landscape and complexity around human health resources.
