Field case management services are also needed in the community to locate and engage homeless individuals who meet the above criteria and to help with identifying available community resources as well as to encourage these individuals to participate in the services offered by the MSC.

To be eligible for placement in the community-based housing unit, all Veterans must be diagnosed as chronically mentally ill, e.g., condition of schizophrenia, major affective disorder, post-traumatic stress disorder, depression, bipolar disorder and anxiety, homeless or at imminent risk of becoming homeless and be eligible and registered for VA services.

The NPLH program provides deferred payment loans to California counties applying independently and and/or with a Development Sponsor for the development of multi-family rental projects with five (5) or more units of Permanent Supportive Housing (PSH) for persons with a serious mental illness who are Homeless, Chronically Homeless or At-Risk of Chronic Homelessness.

The purpose of this contract is to prevent homelessness by assisting chronically mentally ill, homeless Veterans by providing them temporary housing from the street or habitation unfit for Veterans, as well as services necessary for obtaining treatment and permanent housing.
