This project is limited to the Long-Term Care (LTC) area with the intent to utilize the Salesforce platform to create a single repository for LTC Medicaid client case tracking data. The solution will extract data from the State of Vermont (SoV) ACCESS mainframe system to auto-populate Salesforce client case records and combine it with client data input by LTC workers (e.g., resources, name of LTCCC (Long Term Care Clinical Coordinator).

The awardee is expected to work with community resource collaborations such as the Community Reintegration Team (CRT) and Mobile Integration Team (MIT), participate in BPC Round Table meetings, Adult-Single Point of Access (A- SPOA) and Managed Long Term Care options.
