**The Colorado Military Health System (CMHS) requires contractor support to provide non-personal courier services in accordance with the Performance Work Statement (PWS) (RFQ Attachment 1).

REQUIRED CAPABILITIES:The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, other items necessary to perform Military Health Systems Support IT and IM services as defined in the Performance Work Statement except for those items specified as Government furnished property and services.  The Contractor shall perform to the standards in this contract. 

The size standard is $16.5 Million Dollars.The Government intends to purchase support services for Chronic Kidney Disease (CKD) Epidemiology in the Military Health System.   This requirement is in support of the NCCDPHP’s Division of Diabetes Translation (DDT).

This contract will be for services related to the continuing performance of this Project by the Geneva Foundation, for ongoing effort regarding implementation of the intervention as well as dissemination, training and technical input.
