Vermont Health Connect will continue to provide assistance through its Navigator Program for the period July 1, 2024 through June 30, 2025. Navigator Organizations support operations, management, reporting and compensation of Navigators. Navigator Organizations will interact directly with Vermont Health Connect on their work-plan and activity reporting
