The Medical Readiness Contracting Office - West, (MRCO-W), Fort Sam Houston, TX anticipates issuing a Request for Proposal (RFP) to support Brooke Army Medical Center, Joint Base San Antonio to acquire a personal services contract to provide twenty (20) Full Time Nurse Case Managers to provide patient centered, individual, and family level case management.This requirement will be solicited as an 8(a) competitive procurement using Best Value – Trade
Identify problems and develop a plan/education to discuss with the patient regarding any risky behaviors discuss negative consequences of patient actions if needed. Care planning tools: Identify short-term, long-term and ongoing needs of the patient and family and develop strategies to meet these needs. (Use of SMART goals?) Implementing (care coordination): Customize plan of care to meet the needs of the Veteran and caregivers.
