STATE OF ALASKADEPARTMENT OF HEALTH PUBLIC NOTICE The waiver and personal care providers named on the August 1, 2023 Waiver and Personal Care Target Provider list are required, per 7 AAC 145.531(a-h) to submit their annual financial report to the Office of Rate Review within eight months of the end of their fiscal year. Providers not named on the attached list are not required to submit annual financial reports for this reporting cycle.

It is also expected that the provider selected via this RFP process may also provide advisory services as required. 1.2 METHODOLOGY FOR SELECTION All interested providers are being requested to submit required and optional information at one time in the form of a single proposal. All received proposals will be evaluated in stages. The initial stage will entail an evaluation of the technical qualifications described in each proposal.

All providers will be required to submit to a criminal background check, as well as a child abuse registry check to be conducted by the TCJP Human Resources Division. Insurance Note: Coverage for molestation is typically required for services at Gardner Betts Juvenile Justice Center, this coverage has been waived by our Risk Management Office.

Ranked providers may be required to submit price, technical, or other revisions as may result from negotiations or DPW may terminate unsuccessful negotiations and commence negotiating with the next ranked firm. DPW Director will recommend award of the contract based on best value offered to the Planning Department. Contract award will follow receipt of Planning Department’s concurrence on consultant selection and negotiated fee.
