To develop a program of psychosocial support services for Choices for Care (CFC) and MFP participants currently in community-based settings, as well as those transitioning from Skilled Nursing Facilities (SNF’s) to community-based settings, that will enhance on-going self-care and address social isolation of both participants and their caregivers.
Summary The Psychosocial Support Services (PSS) Request for Proposals Application reflects the components each applicant must address to demonstrate expertise and capacity to provide and/or facilitate access to social support for persons living with HIV (PLWH). Proposals must follow the format requirements, address each of the application components, and contain all required attachments to be considered for review.
