Contractor shall provide Temporary/Relief Licensed Physical Therapy, Occupational Therapy, Occupational Therapy Assistant and Speech/Language Pathology Services for patients at DSH-Napa (DSH-N) on an as-needed basis.
The City of Phoenix, Victim Services Division seeks qualified vendors to provide therapeutic services to Family Advocacy Center clients. Vendors must be licensed by the state of Arizona to provide services as a "Licensed Professional Counselor" or "Licensed Associate Counselor."
Contractor shall provide Temporary Relief for Audiology Services and Physical, Occupational and Speech Therapy (POST) Services for patients at DSH-P on an as-needed basis. Services under this Agreement shall be used to temporarily fill vacancies at DSH. This Agreement shall be used only when all other staffing means have been exhausted including, but not limited to, civil service eligibility lists as well as the use of other related Agreements.
