The Contractor shall provide a written report at the conclusion of the service of all passed, repaired, and failed cylinders. If cylinders must be shipped to the Contractor, so that the testing services may be performed, they will be shipped and returned in quantities of 20 cylinders. All Shipping/Transporting Cost shall be included as a separate line item and will be included/added to the Total Quote Price.

Contractors must submit a DBE participation report to the County prior to beginning work on a project and a final DBE report must be submitted at the end of a project. Monthly DBE reports must be submitted with each monthly invoice. Any subsequent changes and/or substitutions of DBE firms from the original DBE Participation Schedule must be submitted and approved by the County in advance.
