The Wisconsin Department of Health Services (DHS) Division of Care and Treatment Services (DCTS) is seeking proposals for nursing staff located but not limited to the seven (7) DHS facilities:
, “Offerors” and/or “Proposers”) to provide patient and staff experience survey and engagement services.
This RFP may result in a single award or multiple awards to contract with the County for the purpose of providing patient and staff experience survey and engagement services as described in this solicitation. It is the County’s intent to award an agreement with an initial term of one (1) year, with an option for the County to renew for four (4) additional one-year periods, unless terminated earlier or otherwise amended.
Travel Expenses. The Contractors travel is included in its cost proposal and the District will not provide separate payment for any travel. 8. Insurance. The Contractors insurance requirements are set forth in the ITB. 9. Confidentiality of Student Records. Contractor understands and agrees that it is subject to all federal and state laws and District rules relating to the confidentiality of student information.
