The major goal of this program is to enhance older adults’ ability to age at home and to reduce social isolation.
Social Isolation Prevention Services for LGBTQ+ Older Adults and People with Disabilities to provide care navigation, peer support and supportive programming to address the emotional, behavioral, health and social isolation challenges faced by LGBTQ+ older adults and adults with disabilities who are isolated. 2.
These entities will pilot services expansion to include education and training about the value of self-care, provide access to mental health supports for individuals and their caregivers, provide access to substance use treatment screening and programs, and reduce social isolation and loneliness of individuals and their caregivers.
