The goal is to have unhoused individuals diverted from the streets to the navigation center. A referral hotline and screening process will be in place, making it necessary for the Homeless Street Outreach team to work collectively with the lead operator of the navigation center to ensure a consistent screening process.

The Program is being implemented in alignment with the City of San Diego’s Community Action Plan on Homelessness, which identifies client-centered, housing-focused outreach as a key component of an integrated homelessness response system. The Program will engage persons experiencing homelessness through two distinct yet complimentary functions: street outreach and street based case management.
