STATE OF ALASKADEPARTMENT OF HEALTH PUBLIC NOTICE The waiver and personal care providers named on the August 1, 2023 Waiver and Personal Care Target Provider list are required, per 7 AAC 145.531(a-h) to submit their annual financial report to the Office of Rate Review within eight months of the end of their fiscal year.
