Shared Health and the Purchasers are seeking Service Providers to develop Community Supportive Living buildings and deliver Community Supportive Living (CSL) services through a pilot in select communities in Interlake-Eastern Regional Health Authority (IERHA), Prairie Mountain Health (PMH), and Southern Health – Santé Sud (SH-SS).
Responses to the Request for Proposal (RFP) must be submitted to Purchasing Department by 10:00 a.m., on Wednesday, March 6, 2024, delivered to 9401 Southwest Freeway, Houston, Texas 77074 in a sealed envelope marked "RFP -6168 Supportive Living Apartments-Property Management Agent Services- DO NOT OPEN UNTIL WEDNESDAY, MARCH 6, 2024 @ 1 0:00 A.M.".
Two additions being explored are a new Supportive Living addition and a new Independent Living addition. The new Supportive Living addition would contain approx. 35 units and would be added on to the northwest side of the existing Supportive Living building. The Independent Living addition would contain approx. 60 units and would be added on to the east side of the existing Supportive Living building.
