To be eligible for placement in the community-based housing unit, all Veterans must be diagnosed as chronically mentally ill, e.g., condition of schizophrenia, major affective disorder, post-traumatic stress disorder, depression, bipolar disorder and anxiety, homeless or at imminent risk of becoming homeless and be eligible and registered for VA services.

This funding will provide enhanced mental health supports for members of our community with serious mental illness experiencing homelessness, including those with substance use disorders, by coordinating outreach services, strengthening partnerships, providing supportive field-based engagement at critical transition times (e.g. upon release from psychiatric hospitalization or incarceration), and reducing barriers to accessing care by providing field-based assessment services.
