This RFP may result in a single award or multiple awards to contract with the County for the purpose of providing patient and staff experience survey and engagement services as described in this solicitation. It is the County’s intent to award an agreement with an initial term of one (1) year, with an option for the County to renew for four (4) additional one-year periods, unless terminated earlier or otherwise amended. The Offeror and the County will agree upon a scope of services outlining the rights and responsibilities of each party through a contract. THIS RFP SOLICITATION REQUIRES ALL PROPOSALS TO BE SUBMITTED ELECTRONICALLY THROUGH PERISCOPE HOLDINGS. NO PAPER, EMAIL OR FAX SUBMISSIONS WILL BE ACCEPTED AND MAY BE DEEMED NONRESPONSIVE.
