The Maricopa County Sheriff's Office, (MCSO), is seeking offers from qualified respondents to provide annual preventative maintenance and repair services for its Philips and Zoll Automated External Defibrillators (AED), and Monitor/Defibrillators.

A) General Requirements:

1) Each unit will be calibrated and checked in accordance with manufacturer guidelines.
 
2) The awarded vendor(s) must be certified to perform this work on the particular manufacturer's unit and will provide documentation upon request. 

3) Annual Preventative Maintenance, (PM), service may be performed onsite at one MCSO location and may

be staged over a few days to complete the work.  Or, units may be picked up and taken to a facility for this work.  If the unit(s) cannot be returned within the same business day, an equivalent loaner(s) will be required. 

B) The fleet consists of the following equipment:

1 - ea.  Philips FRX AED
14 - ea.  Philips FR2 AED
8 - ea. Philips OnSite AED
6 - ea. Philips MRx series ALS monitor/Defibrilator
7 - ea.  Zoll AED+
7 - ea.  Zoll  M Series ALS Monitor/Defibrillator
1 - ea.  Zoll E Series ALS Monitor/Defibrilator
1 - ea.  Zoll X Series ALS monitor/Defibrilator

C) Please attached the following to your offer:

1) Description(s) of the work to be done for each type/model.

2) Service contract form(s), if required, for MCSO review.

3) Optional:  Information on training for MCSO personnel to complete preventative maintenance tasks and/or instructions guidelines for unit maintenance, if a preventative maintenance service is not required.  

D) OPTIONAL: Please attach rate sheets or give an hourly rate for repairs not covered by a PM, (e.g. damage to a unit, etc.)

E) OPTIONAL: Please give any percentage discounts from list price/catalog/web for replacement parts, supplies, etc.
