Primary Care Networks (PCN) Shared Platform - Development Phase Project Description :The Mosaic, Calgary Foothills and Highland PCNs are seeking a company or/an individual(s) to develop the future PCN Shared Platform application that will support both web-based PC and mobile use. A discovery phase was conducted to identify the requirements of the PCN shared Platform that yielded User Stories, Data Dictionaries, and design Mock-ups that will be used to develop the future system. Specifically the system must: - manage administrative and Patient encounter information - manage administrative information about Users of the System, Physicians Members Sites, Primary Care Centers (PCC), Physicians, and Multi-Disciplinary Teams (MDT), - Patient encounter information, such as patient/ provider encounter data - Clinical and non-clinical assessments/forms - Integrated reporting capabilities along with a suite of administrative reports. - The user interface design must consider user story priority level The “Future” state of the system The Development sessions will build the requirements created through the discovery phase. It will also require clear outlines of processes needed to develop, implement, and maintain a new PCN Shared Platform application that is web-based and mobile-friendly. This new PCN Shared Platform system will use for its framework the minimum viable product’s clinical (patient activity) data, non-clinical data, and advancing the patient medical home and reporting information. The high-level requirements and the deliverables for the new system are described below and on subsequent pages.
