Bidder must have at time of bid submittal and for the duration of this Agreement a current and valid:
A. Contractor License(s): A HAZ - Hazardous Substance Removal Certification from California
Contractor¿s State License Board (CSLB).
B. Hazardous Materials Certificate of Registration issued by U.S. Department of Transportation
Pipeline and Hazardous Materials Safety Administration.
C. Hazardous Materials Transportation License issued by California Highway Patrol (CHP).
D. Trauma Scene Waste Management Practitioner (from State of California Department of Public
Health,
Medical Waste Management Program).
E. Waste Tire Hauler Registration issued by Cal Recycle.
F. Hazardous Waste Operations and Emergency Response (HAZWOPER) Certificate of 40
hours of training.
G. Motor Carrier Permit issued by California Department of Motor Vehicles (DMV).
H. Hazardous Waste Transporter Registration issued by the California Department of Toxic
Substances Control.
I. Confined Space Entry Certification, as required by the Department of Industrial Relations,
Division of Occupational Safety and Health (Cal/OSHA), in Exhibit A, Section 16. A permit and
contractors program documents will be required by the start of work.
J. Contractor or subcontractors applying or spraying disinfectant shall possess a Pest Control
Business (PCB) license and have a Qualified Applicator License (QAL) with Category K
(health related) staff to oversee the business operations. Both of these licenses are issued by
Department of Pesticide Regulation.
https://www.cdpr.ca.gov. Contractor or subcontractors applying or spraying disinfectant shall also be registered with each individual county agricultural commission where the disinfectant will be applied (Los Angeles county). Proof of these
requirements shall be provided to Caltrans Contract Manager prior to applying or spraying
disinfectant.
Bidder and any subcontractor(s) must be registered at time of bid submittal with the Department of
Industrial Relations (DIR), pursuant to Labor Code section 1725.5.
