1.1 PURPOSE OF REQUEST FOR PROPOSAL (RFP) The purpose of this RFP is to select a vendor to conduct a monthly review of claims and then deem Medicaid recipients eligible for the Health Home Program. The vendor will also determine the Tier and continuity of Care provider of the recipients using the CDPS system. The vendor will provide a monthly file to DSS with the results. DSS requires other reports as outlined in the SCOPE in section 3.0. 1.2 ISSUING OFFICE AND RFP REFERENCE NUMBER The Division of Medical Services is the issuing office for this document and all subsequent addenda relating to it, on behalf of the State of South Dakota, Department of Social Services. Proposals received after the deadline will be late and ineligible for consideration. Proposals maybe submitted as PDFs via Secured File Transfer Protocol (SFTP). Offerors must request an SFTP folder no later than April 3, 2024 by 5 PM CST, by emailing Kirsten Blachford at the email indicated on page one. The subject line must be RFP10472 SFTP Request. The email should contain the name and the email of the person who will be responsible for uploaded the document(s). Please note, offeror will need to work with their own technical support staff to set up an SFTP compatible software on offeror’s end. While the State of South Dakota can answer questions, State of South Dakota is not responsible for the software required.
