Unusual Occurrence Report (UOR) ToolRFP
NUMBER 210000025Issued August 28th, 2020The Alaska Psychiatric Institute (API) is soliciting proposals for an electronic unusual event reporting tool. The tool will help reduce redundancies in API’s current system by bringing the reporting of medication errors, escalating patient behaviors, patient complaints/grievances into one reporting system.

Prior Experience In order for offers to be considered responsive offerors must meet and provide evidence of these minimum prior experience requirements:
The offeror must have two (2) years in the last five (5) years of experience constructing UOR tools for Healthcare providers.
Reporting system must have at least five (5) years of experience in the past ten (10) years utilized by other healthcare facilities. Link medical records to the reporting system. Each incident must be

emailed to the incident preparer
In offeror's failure to meet these minimum prior experience requirements may cause their proposal to be considered non-responsive and their proposal may be rejected.

The state will not be responsible for any work done by the contractor, even work done in good faith, if it occurs prior to the contract start date set by the state.
