(JPPM). Contractors may utilize their standard bid form, but please include this filled in and signed IFB. In addition, the contractor will submit: Bid/Proposal Affidavit. Conflict of Interest Affidavit and Disclosure. Proof of manufacturer's training. Company's certificates of insurance. Acknowledgement of receipt of any amendment(s) to this IFB. JPPM will regard any bids from contractors without the required paperwork, received after the due date, or received without the completed IFB as non-responsive. Contractors may mail, fax, or e-mail their bids. Bids must be received by the bid due date.Additional InstructionsOfferors must submit a proposal to the e-mail address listed below and be received no later than 3:00 pm on April 19, 2024. Any proposal received after this time and date will not be accepted.
