This requirement is a 100% competitive set-aside for the 8(a) program. Offers from other than 8(a) program participants will not be considered. This
requirement will be a competitive request for quote (RFQ). The RFQ is anticipated to be released on or about 10 June 2019 using solicitation number W912JB-19-Q-0016 and will be posted to FBO. Offerors will be given 30 days to submit a quote from the date of the issuance of the solicitation.The government intends to release a solicitation for qualified 8(a) companies to maintain health readiness records in compliance with Health Readiness Record (HRR). In addition, case management support is needed to manage open cases and referrals. The Performance Work Statement for the technical details will be provided with the solicitation posting. Records tracking will be for the counties of Crawford, Ingham, Jackson, Kent and Wayne for approximately a total of 8,100 soldiers. The resultant award will be a single award firm fixed priced contract for a base period of performance for (8) eight months with two 1-year (12 months) option periods in accordance with 52.217-9. The government will consider an additional extension of up to 6 months in accordance with 52.217-8. Quotes will be evaluated inclusive of options.The award will be based on price, technical approach, past performance. All evaluation factors are approximately equal and an award will be made that will result in the best value to the government. https://www.fbo.gov. All responsible sources that meet the terms and conditions of the solicitation may submit a quote which shall be considered by the agency.
