This notice is for infectious waste collection and disposal services to be provided to the Navajo Area Indian Health Service (NAIHS) facilities located in the Arizona and New Mexico state.Services shall be provided to the following list of facilities:1. Chinle Comprehensive Healthcare Facility; Chinle, AZ2. Crownpoint Comprehensive Healthcare Facility; Crownpoint, NM3. Dzilth-Na-O-Dith-Hle Health Center; Bloomfield, NM4. Four Corners Regional Health Center; Red Mesa, AZ5. Gallup Indian Medical Center; Gallup, NM6. Inscription House Health Center; Shonto, AZ7. Kayenta Health Center; Kayenta, AZ8. Many Farms Dental Clinic; Many Farm, AZ9. Northern Navajo Medical Center; Shiprock, NM10. Pinon Health Center; Pinon, AZ11. Pueblo Pintado Health Center; Crownpoint, NM12. Thoreau Health and Dental Clinic; Thoreau, NM13. Tohatchi Health Center; Tohatchi, NM14.
