The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Other Than Small business firms, to provide a Sound Booth Suite for the Captain James A. Lovell FHCC - Evanston CBOC in Evanston, Illinois. General Requirements: The Captain James A. Lovell FHCC - Evanston CBOC, requires the design, manufacture, and on-site installation of a noise barrier Sound Booth Suite. This Sound Booth Suite shall include a singled wall control room and double walled exam room that will house specialized testing equipment that audiologist utilize when performing audiology examinations to determine hearing loss. General Specifications: Outside dimensions: 16 4
Wide by 9 0 Long by 8 2 high. Shall include the following: - a 36 Wide by 30 High observation window. - Three (3), 20 Wide by 60 High (approximate) windows in the doors. - Cylindrical office function Lock Set with seven (7) pin interchangeable core that will accept a best core. Lock shall always be passage from interior. - Rough in, to hook up to the building HVAC by another contractor. - Fire sprinkler penetrations (sprinklers supplied by others). - LED lights with dimmers. - Four (4) quad outlets in the control room and five (5) quad outlets exam room. - 4 Floor, with vertical 6 Wide filler panels. - To be built to operate on 110v/60Hz unless otherwise specified. * To be constructed on a 4" depressed slab provided by the FHCC.
