The County of Kent, via the Department of Health and Human Services (DHHS) Board, is seeking
proposals that meet this definition. Note: The County is transitioning next year to an October 1 fiscal year. Proposals are for a (9) month budget January 1, 2023 to September 30, 2023. The County will request a second proposal from October 1, 2023 to September 30, 2024, next summer. Proposal Instructions Work Scope: Ensure the proposals include the following: A detailed description of the program, including a client example Program outcomes Details on how the program partners with community solutions Details on how the need was identified and how the program addresses the need Amount requested should be prominently noted on page one of your proposal 3 ~*~ KENT Wwe 2 Budget: Provide a one-page program budget that outlines the budget as follows: Amount Requested: $ Revenue: Source(s) Program Amount List any received or anticipated COVID Related FY 2021/2022 Funding: Source(s) FY 2021 or 2022 Amount Expense: Category Program Amount Staff (inclusive of benefits, taxes, etc.) Contractual Services Rent/Occupancy Equipment Printing Office Supplies Membership Other Total Include a Board approved Agency Budget for the current fiscal year. Length: Proposal (scope + budget) shall be no longer than six pages (size 12 font) in a .pdf version with pages numbered. Attachments: Include a Board roster, program director resume, specific details on other funding opportunities pursued in the past 12 months, most recent year 990 form, and most recent financial audit. COVID mitigation plan narrative: Details on how your agency will be able to provide access to services throughout the pandemic: 3 Proposal Review The Proposal Review Team will make its allocation decisions based on the following rating criteria: The thoroughness of the applicants response to the Request for Proposal Instructions and submission of all required documentation. The Teams assessment of the ongoing need for the specific proposed services. The availability and potential for alternative funding for the proposed service from other funding sources federal, state, local government, etc. Past performance of the agency including specific outcomes and the projects impact on broader goals. In an effort to simplify the application process, the review team will not conduct public hearings for the 2023 Current Unmet Needs Program. It is, therefore, critical that you read the instructions carefully to ensure that your completed application is thorough, complete and accurate; and that you have provided all required documentation. Review members will include the following individuals: Jerry Kooiman Betty Zylstra Bruce McCoy Jennifer James (or designee) Tracey Fountain (or designee) Savator Selden-Johnson (or designee) Other Due to limited funding, no guarantees of continued funding at current levels or increases in funding can be made to past Current Unmet Needs funding recipients. Should the County of Kent, or the DHHS Board, have any questions regarding your proposal, we will contact you for clarification. Otherwise, award notifications will be sent out in mid to late December. The Kent County Standard Submission Terms posted on the Bid Opportunities page of the Kent County Purchasing Division website are incorporated by reference into the solicitation. REQUEST FOR PROPOSAL SUBMISSION Complete submissions must be received in the Kent County Purchasing Division no later than the due date/time specified by the designated clock (local time). Late, faxed, or emailed responses will NOT be considered. The health and safety of the community is a top priority; therefore, Kent County Purchasing Division is shifting all Solicitations to electronic response only until further notice. Reponses may be submitted electronically by selecting the "Submit Online" icon on the Bid Opportunities page of the Kent County Purchasing Division's website. Respondent must include a complete proposal as one (1) non- password protected PDF document, unless otherwise designated by Kent County. The time required to upload a submission may vary. Page number maximums are strongly recommended but not compulsorily disqualifying. Kent County is not liable for costs incurred prior to award. A submission shall constitute an irrevocable offer for a period of sixty (60) days from the due date/time. In the event the notification of award is not made within sixty (60) days from the due date/time, the Respondent may withdraw or provide a written extension of their submission. Submissions may only be withdrawn by written request if the request is received before the due date/time. Withdrawals subsequent to opening shall be subject to Kent County Fiscal Policy Centralized Purchasing 5(i)(2).
