The purpose of this request for proposal is to enter into a contract with a provider of an online platform for the purpose of documenting mental health services (clinical notes) in schools and administration of mental health screenings and/or assessments. The purpose of this request for proposal is to enter into a contract with a provider of an online platform for the purpose of documenting mental health services (clinical notes) in schools and administration of mental health screenings and/or assessments. Weber School District provides school-based mental health services to students in order to assist the student in skill development in order to access their instructional environment and learning. Mental health therapists conduct psycho-social assessments (parent interviews) to collect information relevant to determining appropriateness of services in the school setting. Therapists are also able to implement a systematic process for utilizing approved screening tools to identify the possible presence of one of the approved mental health conditions (R277-625). Additionally, therapists maintain documentation of services provided and progress toward reaching treatment
