Special Requirements: The proposer must submit a package clearly marked on the outside as “Proposal for Streetlight Repair and Maintenance” to the City Clerk’s Office. The package must contain the following: • Four (4) bound copies and one (1) electronic copy (flash drive is preferred method, emails not accepted) of the proposal; • One (1) original and three (3) copies of the cost proposal in a separate sealed envelope clearly marked “Sealed Cost Proposal”; and • A signed W-9.
