instructions outlined below: • CERTIFICATE OF BUY AMERICAN COMPLIANCE FOR MANUFACTURED PRODUCTS – Bidders shall fill out and submit the form with their official bid. • BUY AMERICAN WAIVER REQUEST (If required, the low bidder agrees to prepare and submit to the Owner the B.A. waiver request form within the timeframe specified on the Buy American Certification form in the proposal.) • BUY AMERICAN PRODUCT CONTENT PERCENTAGE WORKSHEET, FAA Form 5100-136 (If required, the low bidder agrees to prepare and submit to the Owner the B.A. content percentage worksheet within the timeframe specified on the Buy American Certification form in the proposal.) • BUY AMERICAN PREFERENCES – FINAL ASSEMBLY QUESTIONNAIRE, FAA Form 5100-137 (If required, the low bidder agrees to prepare and submit to the Owner the B.A. final assembly questionnaire within the timeframe specified on the Buy American Certification form in the proposal.) Addendum 1 – 2 GENERAL CLARIFICATION Question: “Does the furniture that we are specifying fall under the “Buy American” listed? One of the vendors came back and said that only applies to features such as runway lighting etc., and not furniture pieces. Is this accurate?” Response: In the QUOTATION FOR FURNITURE – INSTRUCTIONS TO VENDORS, under Section 1.5 BID PROCEDURES, Bids shall be submitted on the Bid Form (Including the FAA Buy American Forms) included with the Bid Documents. Bidders shall submit the “Certificate of Buy American Compliance for Manufactured Products” with their bid. Additional forms (if required) must be submitted within the timeframe specified on the Buy American Certification Form. Please note, if drop-shipped directly to off-site storage, they will need a Dock and a crew to off-load the semis.” Response: In the QUOTATION FOR FURNITURE – INSTRUCTIONS TO VENDORS, under Section 1.5 BID PROCEDURES, Inspection of merchandise at the off-site storage facility is required to ensure no damage to furniture has occurred in transit. If the awarded vendor selects a drop-shipped delivery method, the awarded vendor is required to have vendor representation at time of delivery to ensure no damage to furniture has occurred in transit and fulfill all requirements outlined in the bidding documents.
