The Service Provider will provide all necessary trucks, cell phones, and personal protective equipment to perform Quality Assurance services on construction projects on an as-needed basis; testing equipment will be provided by the DEPARTMENT. Testing will be performed to ensure projects are in conformance with the plans, specifications and other contract

documents. Work will be assigned at the direction of the DEPARTMENT’s Construction Division. Project Scope:The Service Provider shall provide one (1) Independent Assurance (IA) tester in DEPARTMENT District 1 on an as-needed basis for approximately 80-160 hours per month, with as little as one (1) week’s notice through December 31, 2019.The Service Provider shall provide one (1) IA tester in each of DEPARTMENT Districts 2 and 3 on an as-needed basis for up to 80 hours per month each, excluding winter shutdown period, with as little as one (1) week’s notice through December 31, 2019.The Service Provider shall provide all personnel assigned to this project the proper safety equipment, including but not limited to, soft caps, hard hats and vests meeting the current DEPARTMENT standards for Work Zone Apparel.The Service Provider shall provide a principal engineer to act as Project Manager. The Principal Engineer shall be limited to billing no more than eight (8) hours per month, unless prior approval for additional hours is obtained from the DEPARTMENT.
