Purpose The purpose of this RFA is to provide guidance for current contractors to submit continuation applications to the Iowa Department of Health and Human Services (referred to as Agency) to renew service provisions as described by and within the project period established by the RFP #58821007. Services covered by this application include youth mentoring services (ages 5-18). This program promotes health equity through engaging diverse partners to be involved in and receive youth mentoring services and implementation of Disparate Population Plans to engage and retain disparate youth in mentoring services. Do we need to include administration of pre and post surveys in the work plan? Administration of the Iowa HHS Prevention Survey (pre and post) and associated documents are currently paused. Work plans submitted in the application should reflect this in goals, objectives, rationales and baseline data supporting objectives, and activities. Per page 15 of the RFA, funded programs will be required to participate in evaluation processes as determined by the Agency. An Evaluation Guidance document will be provided, as appropriate, to funded agencies with additional details once determined. Funded agencies will have an opportunity to modify work plans to include evaluation processes, if appropriate.
