Proposer must have the appropriate level of insurance coverage required by the University. University Insurance Requirements are below. Please submit a sample Certificate of Insurance form with your proposal. Insurance must be provided prior to commencement of work 3. Vendors are to acknowledge that once all required information is entered on a sourcing event in the SHSU sourcing system the vendor will have to check the box next to the Certification statement on the Review and Submit section. Once the box has been checked then the vendor will have to click the Submit Response button. If these steps are not followed, the vendor's response will not be

submitted and will not be considered as a valid bid/proposal. 4. Proposer shall agree to terms and conditions contained within University’s Standard Contract Agreement. If Proposer has additional terms and conditions that it proposes to include in any contract or agreement resulting from this RFP (such as software license terms and conditions, participation forms, etc.) or if Proposer takes exception to any terms or conditions set forth in the Agreement, Proposer shall submit a redlined Agreement (in the original editable format, i.e. Microsoft Word) as part of its Proposal. 5. Execution of Offer: Proposer shall complete, sign, and submit the Execution of Offer with their response as an attachment in BearKatBuy Sourcing Director under Prerequisites. The Execution of Offer must be signed by a representative of Proposer duly authorized to bind the Proposer to its proposal.
