INTRODUCTION This requirement is to provide the Veterans Health Administration, Wichita Sterile Processing Service (SPS) and Surgery Department with a service agreement for the maintenance and repair of Steris equipment. 2. BACKGROUND The Wichita VA Medical Center has numerous Steris Equipment that requires full-service maintenance to sustain full operation condition. Contractor must be fully qualified and certified to perform the service requirement. This is requirement is for a Base Year and 4 one-year options, with the Base Year starting on 9/1/2022 to 8/31/2023. 3. SCOPE OF WORK The contractor shall provide all personnel, equipment, tools, materials, parts, supervision, and non-personal services necessary to provide Preventive maintenance and repairs of Steris equipment located at the Robert J Dole VAMC. Equipment listed in Section 4. All equipment listed will have at least a quarterly PM unless otherwise noted. The Contractor shall use OEM parts when performing PM and repairs on the equipment. Contractor shall provide service reports with all work performed per each equipment within 7 days of work being completed. Labor for service calls is covered under the contract. OEM repair parts are the responsibility of the Biomedical Engineering VA personnel. Contractor shall respond to service calls within 24 hours and repair within 72 hrs. Contractor employees shall be competent and have at least 3 years of experience working on Steris equipment. Contractor is responsible of providing any personal protective equipment (PPE) to their employees to complete work. Per the VA Standard Operating Procedures (SOP), the Contractor shall place inspection stickers with the date and due date of the work performed on all the sterilizing and washing equipment. The VA is responsible of providing access to any rooms or equipment where Contractor needs to perform work. Service reports to be given/sent to Biomed department. 4. CONFORMANCE STANDARDS All services rendered under this service contract must be performed in conformance with the National Fire Protection Agency (NFPA), Occupational Safety and Health Administration (OSHA), and Original Equipment Manufacturer standards and specifications. The contractor shall not perform any service that will result in additional charges without prior approval from the Contracting Officer. 6. HOURS OF OPERATIONS (1) Business hours: Monday through Friday, 8:00 a.m. - 4:30 p.m. (2) Off-duty hours: Friday through Monday, 4:30 pm 8:00am; Monday through Friday. (3) National Holidays: The 10 holidays observed by the Federal Government are: New Year s Day; Martin Luther King s Birthday; Presidents Day; Memorial Day; Independence Day; Labor Day; Columbus Day; Veterans Day; Thanksgiving; Christmas; and any other day specifically declared by the President of the United States to be a national holiday. 7. Dole Medical Facility Building 34 5500 East Kellogg Wichita Kansas, 67218 VHA Supplemental Contract Requirements for Combatting COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
