PACIFIC TIME RFP Title: Pest Control for Records Information Center RFP Number: 42224C GS-036 REV 02/26/18 Page 2 of 21 1.0 BACKGROUND INFORMATION The Superior Court of California, County of Santa Clara is seeking qualified pest control entity, person, or company, to provide a quote for pest prevention program that includes but is not limited to monthly prevention services and as needed services focusing on specific problems. 2.0 DESCRIPTION OF SERVICES AND DELIVERABLES The Court seeks the services of a person or entity with expertise in: • Once a month preventative service addressing rodents, mice, ants, cockroaches, and other insects. • Monthly PM includes full site walk interior and exterior of the property, set traps, inspect, and replace exterior traps and

interior traps; replace/change/bait as necessary. • Include a minimum of (20) small roach traps strategically placed throughout the facility. • Include a minimum of (4) exterior rodent trap. • Return services, weekly, if necessary to monitor specific trouble(s) or infestations • Experienced in termite remediation and tenting; priced separately if need. • Services are performed during normal Court business hours; require vendor to designate date/time for monthly service date. The court will provide escort. • Provide supplemental rate sheet for materials, hours of response, and emergency services above the standard preventative services} 3.0 TIMELINE FOR THIS RFP The COURT has developed the following list of key events related to this RFP. All dates are subject to change at the discretion of the COURT. Attachment 2: COURT Standard Terms and Conditions If selected, the person or entity submitting a proposal (the “Proposer”) must sign this COURT Standard Form agreement (the “Terms and Conditions”). Attachment 3: Proposer’s Acceptance of Terms and Conditions On this form, the Proposer must indicate acceptance of the Terms and Conditions or identify exceptions to the Terms and Conditions. Attachment 4: General Certifications Form The Proposer must complete the General Certifications Form and submit the completed form with its proposal. Attachment 5: Darfur Contracting Act Certification The Proposer must complete the Darfur Contracting Act Certification and submit the completed certification with its proposal. Attachment 6: Payee Data Record Form This form contains information the COURT requires in order to process payments and must be submitted with the proposal. DVBE Incentive For vendors claiming the DVBE incentive, the Proposer must complete the DVBE Declaration and Bidder Declaration forms to receive the 3% incentive. See section 13.0. [Only for solicitations of $1,000,000 or more] Attachment 7: Iran Contracting Act Certification The Proposer must complete the Iran Contracting Act Certification and submit the completed certification with its proposal. (Only for solicitations of $100,000, or more) Attachment 8: Unruh and FEHA Certification The Proposer must complete the Unruh Civil Rights Act and California Fair Employment and Housing Act Certification. 5.0 PAYMENT INFORMATION • Payment will be made at the completion of each monthly service. • Payment term is net 30-days after acceptance of service rendered. • The total bid amount is all inclusive. The court will not pay any additional fees. • Pest control or treatments outside of normal prevention services will be treated separately as a project focused on treating of specific issue and will be quoted on a project-by-project basis. • Payments may be subject to withholding if services are not performed or incomplete due to tardiness. 6.0 PRE-PROPOSAL CONFERENCE The COURT will hold a Mandatory pre-proposal conference on the date identified in the timeline above. The pre-proposal conference will be held at the COURT’s offices at 32 Daggett Drive, San Jose, CA 95134. 7.0 SUBMISSIONS OF PROPOSALS 7.1 Proposals should provide straightforward, concise information that satisfies the requirements of the “Proposal Contents” section below. Expensive bindings, color displays, and the like are not necessary or desired. Emphasis should be placed on conformity to the RFP’s instructions and requirements, and completeness and clarity of content. 7.2 The Proposer must submit its proposal in two parts, the technical proposal and the cost proposal. a. The Proposer must submit an electronic version of the entire proposal. The files must be in PDF, Word, or Excel formats.
