Division of Mental Health and Addiction Services (DMHAS) for Level A+ residential services1 for eligible individuals2 with medical needs. This RFP is to develop a level A+ residential program for those with a mental health diagnosis and medical needs discharged from a nursing facility. Total annualized funding for the deficit funded contract is $420,000 each subject to State appropriations. DMHAS anticipates making up to four (4) awards, five (5) beds each. DMHAS anticipates making a total of four (4) awards to provide services to twenty (20) individuals in the following four (4) regions: Region 1 (Sussex, Passaic, Bergen, Warren, Morris, Essex, Hudson) Region 2 (Hunterdon, Somerset, Union) Region 3 (Mercer, Middlesex, Monmouth, Ocean) Region 4 (Burlington, Camden, Gloucester, Salem, Cumberland, Atlantic, Cape May) DMHAS anticipates making one (1) award per each of the four (4) regions identified above. However, if one (1) awardable proposal is not received for each region, multiple
awards in a single region may be considered, but not to exceed four (4) total awards. In addition, up to $600,000 in one-time Capital funding may be requested for the purchase of a single-story home with sufficient bedroom accommodations for five (5) individual consumer residents and the requisite staff. The home must be zoned for residential purposes. Proposals allowing all residents to have single rooms will be awarded fifteen (15) bonus points. Additionally, up to $100,000 in one-time capital funding may be made available for physical modifications to a site either purchased, owned, or in a long-term lease by the bidder. The contract ceiling for the deficit funded contract for services will be up to $420,000 in annualized funding for five (5) individuals. The successful bidder will not be reimbursed for services under a Mental Health fee for service contract. DMHAS is seeking a successful bidder to provide services in a residential setting to five (5) eligible individuals with a mental illness and with medical needs; individuals who are residing in a nursing facility but no longer require that level of care, but continue to have medical needs, shall be given priority. Eligible individuals shall be provided with the requisite rehabilitative services and supports necessary to assist them in achieving mental health rehabilitative and recovery goals, as identified in their individualized rehabilitation plan, in accordance with N.J.A.C. 10:37A-1.1 et seq. The successful bidder will provide qualified staffing and administrative services and will conduct an appropriate schedule of activities to benefit eligible individuals within the residence. Consistent with N.J.A.C. 10:37A, the primary goals of the residential setting will be: 1 See N.J.A.C. 10:37A for Level A+ Program rules. 2 Pursuant to N.J.A.C. 10:37A-1.1, Level A+ services are provided to persons 18 years of age or older with mental illnesses who require assistance, temporarily or permanently, in order to live independently in the community. Restoring a consumer’s level of functioning to that which allows the consumer to achieve community integration and to access an independent living setting of the consumer’s choice. 3. To assist individuals to enjoy an adult life experience that is rewarding, well- balanced, personally enriching, growth-promoting and dignifying. 4. To help individuals restore and rehabilitate functional skills related to social relationships, community participation and membership, self-expression and the responsibilities associated with community living. 5. To support individuals in developing work and leisure activities based on personal preference. 6. To aid individuals in developing a network of valued and supportive personal relationships. 7. To provide mental health assistance, with the addition of medical supports, to the individuals served in this residence. The successful bidder will ensure that the services provided ensure diversity, inclusion, equity, and cultural and linguistic competence to the target population. The successful bidder will continually assess and utilize demographic data of participants’ catchment area in its development and delivery of programming, evaluation, and program outcomes to ensure it is relevant to the population served. Additionally, the successful bidder will analyze data to implement strategies to increase program participation.
